Steam, then the monitors

Sterilization is 21 percent of the outline. The CDC calls saturated steam under pressure the most widely used and most dependable method. The four parameters are steam, pressure, temperature, and time. Recognized minimum exposure periods for wrapped healthcare supplies are 30 minutes at 121°C (250°F) in a gravity displacement sterilizer or 4 minutes at 132°C (270°F) in a prevacuum sterilizer. The cycle you actually run is the one in the sterilizer and device IFUs.

A dynamic-air-removal sterilizer gets a Bowie-Dick test each day it is used, before the first processed load, in an otherwise empty chamber. A failure means you do not run loads until the sterilizer is checked and passes. Steam biological indicators use Geobacillus stearothermophilus. A positive biological indicator is a failed process until it is investigated: operator error, steam delivery, or the machine.

Release, implants, and failures

Read the physical monitors (time, temperature, pressure) and sign the printout. Loads with implants stay quarantined until the biological result is known, except in the rare documented emergency the facility policy allows. Immediate-use steam sterilization is not a convenience cycle and not a substitute for buying another set. The CDC does not recommend it for implants; if it is unavoidable, the load, the patient, and the biological result have to be traceable.

Low-temperature methods (ethylene oxide, hydrogen peroxide gas plasma, liquid chemical sterilization) exist for devices that cannot take steam. The packaging, the biological indicator, and the aeration or immediate-use rules come from that system's IFU. A liquid chemical process does not give you a packaged shelf-stable sterile product.

Question 1 of 5

Sterilization Process

What is the recognized minimum exposure for wrapped instruments in a gravity-displacement steam sterilizer at 121°C (250°F)?

Common questions

What spore monitors steam?

Geobacillus stearothermophilus. Do not swap in the indicator from a different sterilization method.

What do I do with a failed Bowie-Dick test?

Do not use the sterilizer for loads. Have it inspected and repeat the test until it passes.
